COVID-19 vaccine information

Vaccine administration at ADMH is by APPOINTMENT ONLY and is limited to:

individuals age 18 and up who have developmental or intellectual disabilities plus a maximum of two caregivers per household (for those households where two caregivers have not already received vaccines)

the parents of children with complex medical conditions

A letter of medical necessity from your physician will be required if you are not a current ADMH patient.

Appointments will be made for vaccine administration as vaccine becomes available and as per administration guidelines from the Georgia Department of Public Health.
